    Please request refills at appointments, through your pharmacy, or through the patient portal.

  • Can I have labs done at your office?

    We can collect labs in the clinic (these need to be scheduled).

  • How can I get the results of my test?

    We review results within an 8 to 10-day period. You will be notified by phone or patient portal.

  • How do I get referrals for specialists?

    If your provider determines a necessary referral, this will be completed in the office. If referral is requested, there will be a fee of $25 for staff time and efforts.

  • Is there a fee for filling out forms or letters?

    Yes. Form completion and requested letters will cost $25 per occurrence based on the complexity of the form or provider time involved. Please allow 7 to 10 days for us to complete forms or letters.

  • Who do I talk to if I have billing questions?
